You'll need:

• Most recent report card
• Latest AIMS* test scores
• Birth certificate
• Immunization record
• Description of any special medical concerns
• If applicable, Arizona State Child Find form [40kb Adobe PDF]

Remember: safety first, the safety of your child and the safety of every one of our students.

Bring your child’s most recent report card and AIMS* test scores. We can’t place your child in the right level without the AIMS scores. If your child did not take the AIMS test due to living in another state, please bring his or her most recent standardized test of any kind. Your school is required by NCLB (definition: No Child Left Behind) to provide you with your child’s test scores.

We'll need a birth certificate and an immunization record. (Yes, we can see that your child has been born. A birth certificate is simply a government requirement.)

We need to know as much as possible about your child in order to provide the safest environment for him or her. Start with the back of the pre-enrollment form, where we ask if your child is working on grade level or not; where we ask if your child has ever been referred for special education and if he or she does have an IEP (definition:Individualized Education Program).

Does your child have a medical condition of which we need to be aware? Please tell us about that in writing and request that the teachers be made aware of any condition that might impact your child’s learning or safety.

In order to facilitate the application of Arizona's Child Find program - that children with delays or disabilities receive the supports and services they need - fill out the Child Find tracking form.

* AIMS is the acronym for Arizona's Instrument to Measure Standards, the State's educational testing standard for primary and secondary schools (grades K -12).
